Old Students

Present the following:

     1.  F138/SF09 (Report card)

     2. 2 copies 2 x 2  ID colored pictures for issuance of application card and

        registration forms.

     3. 1 long brown envelop (Grade VII only)

 

New Students and Transferees

Entrance examination and interview.

     1.  The applicant must pay an entrance examination fee at the cashier’s office.

     2. Take the entrance test.

     3. Transferees students must have an average of 82 or above

     4. Wait for the result. Interview with the student and parent will follow.

     5. Submit to the registrar’s office the following requirements.

            NEW STUDENTS: 

                Colored Photocopy of Birth Certificate PSA

                Medical Certificate (with Medical Conditions)

                2 pcs. Colored ID picture (2 x 2)

                1 Long Brown Envelop

 

           TRANSFEREES:

               Entrance Exam

               F138 / SF09 (Report Card) with LRN

               F137 / SF10 / ECCD (upon request)

               Certification of Completion - Incoming Kinder 2 & Grade 1

               Good Moral Character

               Colored Photocopy of Birth Certificate PSA

               Medical Certificate (Student w/Medical Conditions)

               2 pcs. Colored ID picture (2 x 2)

               1 Long Brown Envelop

     5.  Proceed to the cashier to pay tuition fee, books and uniforms.
